Tips for Not Running Out of Your Meds
It is very important to ensure that you have adequate supply of your long-term medications for conditions such as diabetes, high blood pressure, heart disease, asthma, seizures and blood thinners.
Please contact the office 1 week before completion of your final refill. Press option 1.
Call us before the weekend or holidays when office might be closed.
You can also have your pharmacy contact our office.
Use the patient portal for faster response. Avoid telephone calls and phone prompts.
Set reminders on your phones to let you know when refills are due.

We take steps to protect you against COVID19!
During the COVID19 pandemic, we are starting to see patients in the office for care. We do not take walk-ins and we encourage all patients who are ill to call the office first. We are taking steps to keep patients and staff safe, including requiring social distancing, mask wearing of facial covering and sanitizing regularly. While at the office, we stress it is important to wear your mask at all times, even as you wait in the exam rooms.
Most of the other appointments are by telehealth, unless it is determined that there must be an in-person evaluation. To reduce unnecessary traffic flow, please utilized our patient portal system and telemedicine visits.

Your Patient Care Medical Home. At PHA-Adult Medicine, we deliver care with the patient's needs as the focus. The doctor-patient relationship is a partnership. We have adopted the patient-centered medical home model to better coordinate care. We encourage couples to come together at appointments and for family members/care givers of the elderly to be actively involved. To better serve our community, we have popular SAME DAY appointments, urgent care appointments as well as appointments for follow-up care and well-exams. The patient portal. allows 24-hr access to medical records. We encourage all patients to utilize the patient portal. All you need is an email address and internet access with computer, tablet or smartphone.
As your patient care medical home, we encourage you to Call US First before going to ER. Often we can address your medical concern in the office or by telephone. We save you money and time.

At least once a year we perform health risk assessment on our elderly patients in order to identify needs and health risks. We examine physical, emotional and mental well-being, looking at cognitive decline, fall risk and social needs.
