We have adopted the patient-centered medical home model to better coordinate care. We encourage couples to come together at appointments and for family members/care givers of the elderly to be actively involved. To better serve our community, we have popular SAME DAY appointments, urgent care appointments as well as appointments for follow-up care and well-exams. The patient portal. allows 24hr access to medical records.  We encourage all patients to utilize the patient portal. All you need is an email address and internet access with computer, tablet or smartphone. We provide services for acute medical conditions as well as coordinate care for chronic illnesses, Whether you want a physical for your job, college entrance, driver's permit or preventative health, we can provide that for you. Do you plan to have surgery and need medical clearance? See us within 30 days of your surgical date.
