Serving Our Elderly

At least once a year we perform health risk assessment on our elderly patients in order to identify needs and health risks. We examine physical, emotional and mental well-being, looking at cognitive decline, fall risk and social needs

To better deliver high level optimal care to our elderly, we are active member of Constitution Physician Alliance. This is an independent physician association of dedicated private community physicians working to deliver preventive care, coordinated care and cost-effective care to our combined elderly population.

After being hospitalized or visiting the ER, we encourage our member to contact us without 24-48 hours to review discharged medications and to see us in the office. We can do so via telemedicine or in-person. This connection is very important to reduce readmission due to recovery issues or confusion about discharged medications.
